Onsite Women's Health (Mammography) Data Breach Settlement

U.S. residents whose personally identifiable information (PII) and/or protected health information (PHI) may have been impacted in the Onsite Women's Health (Onsite Mammography) data incident, including everyone who received a breach notice. Onsite Mammography, LLC, doing business as Onsite Women's Health (a provider of in-office mammography and women's-health screening services), reached a $2.53 million settlement over a data incident that potentially exposed patients' PII and PHI. Class members can submit a claim for a pro rata cash payment with no documentation, reimbursement of documented out-of-pocket losses, or credit and medical/health-data monitoring services. Onsite denies wrongdoing.

Who's excluded: Class members who opt out by the deadline give up all benefits but keep the right to sue separately. Only U.S. residents whose information was impacted in the incident (those who received notice) are covered.
