Calibrated Healthcare Data Breach Settlement

The roughly 34,562 people notified that their information was compromised in Calibrated Healthcare's February 2024 data breach. You automatically receive about $21–$29 plus two years of medical and identity monitoring with no claim form; filing a claim adds up to $175 for documented time (7 hours at $25/hr) and up to $5,000 for documented out-of-pocket losses.

Calibrated Healthcare Systems, LLC suffered a data breach in February 2024 in which files containing patients' personally identifiable information (PII) and protected health information (PHI) were accessed and disclosed without authorization. About 34,562 people were affected. Under the settlement, every class member automatically receives roughly $21.44–$28.68 in cash plus two years of Medical Shield Total monitoring — no claim required — and can additionally claim up to $175 for documented time and up to $5,000 for documented out-of-pocket losses. Calibrated denies wrongdoing.

Who's excluded: Class members who opt out by July 9, 2026 give up the automatic cash and monitoring and any other benefits. Only people notified of the breach are covered.
