Living U.S. residents whose personal information (e.g. driver's license number) was compromised in the Lemonade quote-platform data exposure between April 2023 and September 18, 2024 — about 190,000 people who were sent breach notices in April and June 2025.

## How to file your claim
1. Open the official claim form at lemonadedatadisclosuresettlement.com and enter the Class Member ID from your notice.
2. Choose the pro rata cash payment and/or upload documentation for losses up to $10,000.
